Our practice has been selected to participate in the National COVID-19 Vaccination Program.
In late April 2021, our clinic will start by receiving 50 doses of the AstraZeneca vaccine per week, so patience will be required by all. If you are interested, please speak with reception to place yourself on our clinic’s waiting list for the vaccine. Due to supply restrictions, we are currently only able to offer COVID vaccines to existing patients.

All patients MUST complete a Vaccination Consent Form prior to be vaccinated against COVID-19.

Alternatively, our reception staff are able to provide you with a hard copy.
Once submitted, our staff will decide if it is appropriate to book you in to one of our dedicated vaccination clinics. If we decide your situation needs to be reviewed, or if you need more information before you feel confident making an informed decision, you will need to book a regular appointment to discuss.
