COVID-19 Vaccine Information


From July 2021 BHWC will cease administering first doses of the AstraZeneca Covid-19 Vaccine. To find what your options for vaccination are, please read the statement on our Home Page

Eligibility

In order to find out when you can receive a COVID-19 vaccine at our Practice or anywhere, you may use the tool below. The link will navigate you to the COVID-19 Eligibility Checker tool on the Department of Health Website.

Consent

All patients MUST complete a Vaccination Consent Form prior to be vaccinated against COVID-19.

Please follow the link below to fill out the Consent Form prior to coming in.

Alternatively, our reception staff are able to provide you with a hard copy.   

Once submitted, our staff will decide if it is appropriate to book you in to one of our dedicated vaccination clinics.  If we decide your situation needs to be reviewed, or if you need more information before you feel confident making an informed decision, you will need to book a regular appointment to discuss.
